How to Build Forms

Learn how to create beautiful forms using our drag-and-drop builder. From connecting to Quickbase to publishing your form.

Step 1: Connect Your Quickbase Account

  1. 1Navigate to Connections in your dashboard
  2. 2Click + Create New (or Create New) to add a connection
  3. 3Enter your QuickBase Realm (e.g., yourcompany.quickbase.com)
  4. 4Enter your User Token (from QuickBase: Profile → Manage my user tokens for realm — see Managing Connections for details)
  5. 5Give your connection a name and save

Your Quickbase credentials are encrypted and stored securely. You can create multiple connections for different Quickbase accounts or apps.

Step 2: Create a New Form

  1. 1Click Create New Form from your dashboard
  2. 2Enter a Form Name and optional description
  3. 3Select your Quickbase Connection
  4. 4Choose the App and Table where submissions will be saved

Step 3: Add Fields to Your Form

The form builder makes it easy to add fields from your Quickbase table. Here's how:

  • QuickBase Fields: In the right sidebar, the list of QuickBase fields is sorted A–Z and can be searched. Click a field to add it; these fields write to your table when submitted.
  • Custom Fields: Add custom fields like paragraphs, images, or buttons that don't map to QuickBase fields.
  • Drag to Reorder: Click and drag fields to rearrange them in your form.
  • Field Settings: Click any field to customize its label, placeholder, required status, and more.
  • Switching Tables: If you change the QuickBase table for the form and you already have fields, you'll be asked to confirm; confirming will clear existing fields so you can rebuild for the new table.

💡 Pro Tip: Reference Fields

If your Quickbase table has reference fields (parent-child relationships), you can add lookup proxy fields. These will automatically query the parent table and display user-friendly values while submitting the record ID.

Step 4: Customize Branding & Styling

Make your form match your brand perfectly:

  • Colors: Set page background, form background, header color, submit button color, header text color, and submit button text color (text colors default to white)
  • Logo: Upload your company logo to display at the top of the form
  • Font: Choose from a variety of fonts or use your custom font
  • Corner Style: Round or square corners for your form
  • Submit Button: Customize button text and color (and button text color), or hide it entirely. For custom Button fields, you can set the button text color in that field's settings.

Step 5: Preview Your Form

Before publishing, always preview your form to make sure everything looks and works correctly:

  • Click the Preview button in the form builder
  • Test all field types and validation
  • Check that fields are required when they should be
  • Verify branding and styling look correct
  • Test on mobile devices to ensure responsiveness

Step 6: Publish Your Form

Once you're happy with your form, it's time to publish:

  1. 1Click Save Form in the form builder
  2. 2Choose whether your form is Public (anyone can access) or Internal (requires authentication)
  3. 3Copy the form URL or embed code
  4. 4Share your form link or embed it on your website

✅ You're Done!

Your form is now live and ready to collect submissions. All submissions will automatically be written to your Quickbase table.
